Summarize the progress youâ€™ve made to date plus your plan for the upcoming week. Include the components listed below. Note that most sections require a list rather than a paragraph.
- Goal: Re-state the goal from your Health Behavior Change Contract. If necessary, based on the feedback provided for the assignment, revise your goal statement. This should be stated in a complete sentence
- Strategies Used: List at least 2 strategies that you used over the past week, labeling each as either successful or unsuccessful.
- Strategies for Upcoming Week: List at least 2 strategies you plan to implement in the upcoming week to help you achieve your goal. These may include, but are not limited to, successful strategies identified in the previous section.
- Obstacles: List at least 1 obstacle that you anticipate encountering in the upcoming week as you work towards achieving your goal.
- Strategies for Overcoming Obstacles: For each obstacle, list at least 1 strategy you can use to minimize, or overcome, each obstacle listed above.
